Trout Camp Registration Form

Welcome to Trout Camp Registration!

Please complete the registration form below to enroll your child in Trout Camp. After submitting the form, you will be redirected to our Square Payment Processing for payment. Once payment is confirmed, you will receive a confirmation email from our Trout Camp Coordinator.

The cost is refundable, minus $25 to cover credit card fees and servicing, if the participant cancels on or before May 15.

We have a limited number of Redington Crosswater Fly Rod Combos available for $128.75.

Please note that Trout Camp provides all necessary equipment, so there is absolutely no requirement to purchase gear. However, if you’re interested in getting started with fly fishing at home, the Crosswater Fly Rod Combo offers nearly everything a beginner needs to get up and running.

Permission & Agreement
In consideration of my child’s participation in the Teen Trout Camp of the Utah Chapter/Council of Trout Unlimited ‘Activities,’ the subscribing parents or legal guardians of said child hereby provide their voluntary assent to the child’s engagement in the Activities. The subscribing parties acknowledge and fully comprehend that the Activities entail inherent dangers and risks, inclusive but not restricted to the potential for severe physical injury, fatality, or other adverse outcomes that may arise directly or indirectly from participation in the Activities.

The undersigned hereby voluntarily elect to permit our child to participate in the Activities and assume all risks that may be associated with such Activities and release and hold harmless Trout Unlimited and its officials, agents, officers, employees and volunteers of and from any and all liability, claims, demands, actions, and causes of action arising out of or related to any injury, including death, that may be sustained by our child or to any loss or damage to any property of our child while participating in the Activities.

The undersigned grants the above listed parties the right to use photographs of the Child, or which involve the Child and his/her participation in the project, and we release the above parties from any liability stemming from the use of any such photographs.

This release shall be binding upon the heirs and personal representative of the undersigned.
I further agree that my child will abide by the rules and directions given by Trout Unlimited during the Activities.

If any provision or portion of this Agreement is held to be unconstitutional, invalid or unenforceable, the remaining provisions shall remain in full force and effect.

I represent that I am the parent and/or legal guardian of such child; I agree to indemnify the released parties for any and all claims whatsoever brought by the child; and I agree to indemnify the released parties for any and all claims whatsoever brought by a third party arising out of the child’s conduct.

I HAVE READ THE ABOVE WAIVER AND RELEASE, UNDERSTAND THAT I MAY GIVE UP SUBSTANTIAL RIGHTS BY SIGNING IT AND SIGN IT VOLUNTARILY WITHOUT INDUCEMENTS.
